The process is easy:
Download & complete the Agency Profile
Download & complete the Retail Producer Agreement
Download and complete the W-9 form: http://www.irs.gov/pub/irs-pdf/fw9.pdf
Submit with a copy of your license and E&O Declarations Page to your local XPT office. Upon receipt of all of the necessary material listed above, we will begin processing your appointment.
